ADDITIONAL SERVICES FOOD HANDLER CERTIFICATE
Food handlers or food vendors medicals is done solely by the District Assembly. The Prampram Polyclinic has a laboratory with qualified laboratory technicians assigned to handle Food handlers. NOTE: This process takes two(2) weeks to complete and only medically fit clients will be issued with the food handler's card.
COMPLAINTS OF NOISE NUISSANCE
Complaints shall be made at the District Environmental Health Department. A letter indicating the area, the address and signature of the affected persons shall be added as supporting documents.
DESTRUCTION OF UNWHOLESOME ITEMS
Application letter addressed to the Director specifying the items to be destroyed and quantity.
DISINFECTION AND DISINFESTATION Application letter addressed to the Director. To obtain a grave space, you will need to the medical cause of death from an approved health institution.
CREMATION OF CORPSE One would need
1. Application letter addressed to the Director.
2. Cause of death
3. Embalmment Certificate (specifying the chemical used).
4. Proof of membership of cremation society (if a Ghanaian).
5. Photocopy of passport (if a foreigner).
6. Payment is made through the approved systems in accordance with the gazette fee.
NB This is done in a day.
EXHUMATION One would need
1. Application letter addressed to the Director (District Environmental Health Deparment).
2. Court order (from a higher court).
3. Copy of burial permit.
4. Death certificate.
5. Cause of death.
6. Identification of the deceased person.
7. Identification of grave or burial place.
8. Payment is made through the approved systems in accordance with the gazette fee.
PAUPER BURIAL/UNCLAIMED BODIES One would need
1. Police (Coroner's certificate from court).
2. Letter from Police or verbal complain to the Director
NB All pauper cases should be reported to the Police.
MASS BURIAL One would need
1. Application letter addressed to the Director (District Environmental Health Deparment).
